* Weekly appointments available

* We require complete copies of the medical record in order to provide a comprehensive report.. Complete medical record includes office notes, physical therapy notes, operative notes, xray reports, diagnostic reports, xray & MRI films.

* To request an IME, print the IME Request form and fax to our office at (770) 479-9491. We will contact you within 24 hours with an appointment date & time.
